Factor Analysis Associated with Lymph Node Metastasis in 1301 Early Gastric Cancer



Abstract
A retrospective analysis of 1301 cases of EGC had been performed to evaluate its prognostic factors. The parameters that we studied were sex, age, location, histology, extent of resection, depth of invasion, gross type, lymph node metastasis,
size
of
the lesion. In univariate and multibariate analysis of the nine factors, statistically significant prognostic factor was the status of lymph node metastasis only(P<0.001), but others had no association with prognosis statistically. And then we
tried to
identify the factors associated with the lymph node metastasis. The rate of lymph node metstasis analyzed; the association with location of cancer, depth of invasion, gross type, size of lesion and histologic type. The rate of lymph node
metastasis
was
high, in antral lesion, submucosal lesion, protruding(I and IIa) type, larger sized lesion and poorly differentiated type. But in signet ring cell type, the rate was low. Statistically, the risk factors for lymph node metastasis were found to be
the
protruding type and submucosal involvement. Others mentioned above had no significance statistically. But protruding and well differentiated mucocal cancer(79 cases)revealed low rate of lymph node metastasis(at most 3.8%). In conclusion,
cxtensive
lymphadenectomy is necessary in the surgical treatment of EGC till now and in selected cases, limited surgical procedure may be considered.
